Provider development and formation of a GP collaborative.

Practices had differing understanding of the need for change and several were not keen to participate. Many were concerned about the sustainability of general practice. 

GPs needed a way to participate in the formation and delivery of whole system Alliance of providers and commissioners 

Our work:

We worked with and developed a core team of primary care leaders who would lead the collaborative – morphed into formal board later.

Scheduled regular all practice engagement events and worked on reaching an agreement with more than 50 practices to form an alliance / collaborative where all practices paid an initial sum to get it started.

Led development and delivery of Integrated model of care and actively participated in subsequent transformation in community.

Outcomes:

·       A viable vehicle for practices to work collaboratively and contract at scale

·       Success in winning Integrated Care Urgent care Bid in partnership with local acute trust – currently year 2 of operation  

·       Core member of the commissioner-provider alliance and led / participated in whole system transformation 

·       Development and Delivery of MDT Integrated community networks around primary care 

·       Delivery of a number of other projects such as HSJ finalist MSK pilot, Working at scale with local pharmacies (One Primary Care) and Enhanced access
