Development and delivery of an integrated model of care for older people for a STP/ICS.

Problem:

The Health System had a large financial gap projected to increase to more than £150M in the next 10 years. Commissioners adopted an outcomes based commissioning approach to address this but needed assistance with designing a new model of care with integrated care networks.

Our work: 

We were the SRO with responsibility of designing the model of care ensuring stakeholder engagement across the system. The model of care was built around integrated care networks, centred on proactive management community networks.  

Outcomes:

 A transformational person-centred model of care capturing best practice and an Alliance of 5 providers was formed to deliver it.
