Redesign of community mental health pathways for an ICS.

Problem:

NHS Patients in the boroughs  were experiencing significant issues accessing mental health services with long waiting times and lack of engagement between agencies. 

Our work:

We were commissioned to assess causation and to lead the re-design and mobilisation of new integrated care pathways.   This involved engaging with the whole system including local authorities, public health, PCNs, secondary care and ICS leaders.

This project was enabled by an educational approach of understanding competing pressures in the patient journey, the effect on clinical outcomes and demonstrating the benefits through multidisciplinary education.

Outcome:

The re-designed integrated community mental care pathways have been successfully rolled out, launched by a spontaneous round of applause at our final presentation!
