Our work:

  • Developed a cross-system think tank including Healthwatch, council, CCG, and front line staff to decide on a model of care

  • Brought a unique methodology to support an approach that can be used for any long term condition

  • Supported the creation of a diabetes dashboard to support the QI cycle

Outcomes:

  • Built on cross system working during covid and developed strong working cross system

  • Developed a methodology to use in other transformation work

  • Gathered detailed insights from the population, data and professional opinion to design a tailored and bespoke model of care
