Development and Delivery of an integrated model of care for older people

The Background:

The Health System had a large financial gap projected to increase to more than £150M in the next 10 years. The local health and care commissioners adopted an outcomes based commissioning approach to address this but needed assistance with designing a new model of care and integrated care networks.

The Work:

Ash was the SRO with responsibility of designing the model of care ensuring stakeholder engagement across the system. A transformational person centred model of care capturing best practice was developed and an Alliance of 5 providers was formed to deliver it. The model of care is built around Integrated Care Networks and aimed at proactive management and galvanising community networks around people and family.

The Impact:

The new model of care was delivered within the agreed timescale and received acceptance from all relevant stakeholders. A return on investment review was performed 12 months after the introduction of the new model of care and results showed a 6% reduction in non-elective admissions for over 65’s. The Alliance was responsible for services worth £180M p.a. and demonstrated it returned £1.44 for every £1 spent on the ICN programme and £2.67 for every £1 invested in Life (Living independently for everyone).
