Development of a PCN Neighbourhood Development & Delivery Plan.

Problem:

To create a template for a ‘PCN Development & Delivery for other CCGs within the STP. 

Our work: 

The CCG was already delivering an advanced local integrated care mode however, there   were high levels of internal variation with engagement, motivation and developmental between the CCG’s constituent PCNs. We explored the local system and contexts through engagement with key stakeholders and service users, leading a team made up of CCG and federation staff. 

Outcome:

A final report describing;

• The risks of strategic dissonance between similar but competing local strategies that were holding PCNs back from engaging with the PCN DES agenda.

• The need to balance bottom-up delivery of change in practice by PCNs with top down support 

• A tangible delivery plan broken down by initiative, using a framework of ‘culture, capability and capacity’
