Multi-practice merger.

Problem:

Deprived urban area with a large practice surrounded by 3 single handed GPs all close to retirement.

Our work:

• Building on existing working relations to a point where merger could be contemplated

• Writing the business plan for the merger to submit for CCG approval and acting as the operational plan for the practices

• Developing the operating structure, IMT and workforce model for a single-point of access for patients

• Managing extensive patient engagement

Outcome:

• Successful application to CCG

• Practice with single list oc circa 20kpatients operating across 4 sites with a single point of telephone access staffed by an MDT.
