Development and delivery of an integrated urgent care service.

The Background:

The health system was struggling to manage a high number of attendances in A&E and urgent care plus increasing non-elective admissions to hospital and developed an integrated urgent care pathway.

The Work:

Ash and Andrew worked together on the design and development of an integrated solution to deliver the proposed pathway including a UCC, a network of GP led Health Centres and out of hours services. The project approach involved extensive provider and GP engagement and the delivery solution developed was supported by the local GP federation, Practices, the local Acute NHS Trust, mental health NHS Trust and a leading Third Sector organisation.

The Impact:

The solution was successfully rolled out and continues to be delivered with the support of the local GPs.
