Integrated Care Systems

Integrated models of care development.

Through our experience we recognise that a single intervention can not have an impact without understanding system synchronicity. We engage with stakeholders to co-produce models, informed by an understanding of joint system priorities and parallel workstreams.

We implement end to end transformation, embedding preventative measures for long term sustainability.

Demand and capacity modelling.

Our demand and capacity modelling uses advanced data interrogation, which helps to:

a. Quantify the changes needed to manage services effectively.

b. Inform strategic decision making and planning.

c. Support delivery of timely care to people.

Robust demand and capacity modelling improves waiting times, patient choice and experience for service users and staff.

Understanding drivers of demand.

Using data intelligence including: bench marking; trends; population health; and local professional insights, we convert data into intelligence, through multi level data interrogation. At each step we ask the question – ‘so what?’ – which builds a case for change in order to:

a. Enable professionals to get a better understanding of key areas of focus within the system

b. Develop a whole system model of care

c. Bring stakeholders together in order to drive delivery

Whole systems pathway development.

We combine the skills of our expert clinical associates, often leaders and innovators in their own right, with our change management expertise to provide end to end clinical and operational optimisation of care pathways. Often this involves de-medicalising the approach, making best use of existing and community assets, harnessing creativity and implementing technological solutions.

We bring experience, insights, expertise and engagement to commissioners and providers of care when they are searching for solutions to redesign and rethink care delivery systems.
